Key:
# - Has replaced a number (e.g. 1 or 2 or 3 etc...)
X - Has replaced a capital letter (e.g. A or B or C etc...)
x - Has replaced an uncapitalized letter (e.g. a or b or c etc...)
(d/m/y) - To indicate the order of date
Xxxxx - Replaced a word which begun with a capital letter (e.g. Fish)
XXXX - Replaced an all-caps word (e.g. FISH)
(NHS number) - To indicate where the NHS number was placed in its format of 'XXXXX ###' (e.g. ABCDE 123)
##th - To indicate the format of date (e.g. 5th or 7th or 19th etc....)
